INFORMED CONSENT FORM FOR RESEARCH BEING CONDUCTED AT NORTHEASTERN STATE UNIVERSITY
Title of Project: Religious Ideologies
Principal Investigator(s): This study is being conducted by Lucas Driezen (driezen@nsuok.edu) under the direction of Dr. April Phillips (philli48@nsuok.edu).
Purpose of the Study: The purpose of this study is to examine how people’s religious beliefs influence their attitudes and behaviors in certain situations.
Procedures to be Followed: This study will be conducted as an online survey. You will be asked to complete a very brief survey.
Discomforts & Risks: There are no anticipated risks or discomforts beyond those experienced in everyday life associated with your participation in this study.
Costs or Compensation: Participation in this study is completely voluntary. There are no costs associated with your participation in this study. If you are a student at NSU, your instructor may offer you extra credit for participating in this study. The amount of credit offered for participation will be up to your instructor. If you have questions about how the credits will be applied to your grade please contact him/her. If you are participating in this study for payment via MTurk your payment will be processed upon completion of the study. Participants that have questions about MTurk should contact the researchers listed above. If you start the study and decide to withdraw for any reason, you will not be penalized in any way.
Duration: It should take no more than 30 minutes to complete this survey.
Statement of Confidentiality: This survey is anonymous meaning we will not be recording your name or any identifying information about you. Findings from this study will be presented in aggregate form so that individual participants will not be identifiable. If you are receiving credit in a course for participating in this study, you will be able to print a confirmation page after completing the survey that you can give to your instructor to document your participation. Your instructor will only know that you participated in this study. He/she will not have access to your data.
Right to Ask Questions: You have the right to ask any questions during and after you participate in this study. Participants may contact the researcher or faculty sponsor listed above with any questions about the study. You may also contact the Institutional Review Board at Northeastern State University at (918) 444-2917 or irb@nsuok.edu.
Voluntary Participation & Right to Withdraw: Participation in this study is completely voluntary. You have the right to refuse to participate. You also have the right to withdraw from the study at any time without penalty. Participants who chose to withdraw from the survey can do so by not submitting their responses and closing the web browser. Once responses have been submitted, participants will not be allowed to withdraw due to the anonymity of the survey. You also have the right to refuse to answer any individual questions without withdrawing from the entire study.
