Registration courses 2019-2020

Fill in this registration form only when you have got an Erasmus + grant for one or more courses for school year 2019-2020 . See next-to-last question if you want to register for courses of school year 2020-2021 (courses from August 2020 onwards).
Do not fill in If you are applying for a grant or still waiting for approval of your grant. Fill in only after approval of your grant or if you have other funding.
If you have other funding for coming to one or more Euneos courses, please mention in question 1.
Questions 1, 2, 3, 4, 6, 7, 8, 9 and last one are compulsory, so you have to check that you fill in these questions.
IMPORTANT. Read all the questions before you start filling in and check if you have all requested information ready.

It is no problem if you don't know yet the names of the participating teachers. You can fill in teacher 1, teacher 2 etc. After selection of the teachers you send the names and mail addresses by mail to courses@euneos.eu.
Please do not fill in this SurveyMonkey registration form more than once.

IMPORTANT

For our database we prefer to have only 1 registration per school. Please check in your school if someone has already made a registration for Euneos courses for school year 2019-2020 and mention it when you make your registration.
 
For all changes in existing registrations (change of participant, change of course, cancellation) please send a mail to courses@euneos.eu You cannot replace participants without informing us.

Registration of big groups (more than 12):
When you want to register more than 12 participants of your school, there are 2 options:
- if there are only a few more than 12, you can fill in their names and chosen course in the comment field of question 11.
- if there are many more you can split the group and make 2 registrations. Mention in the comment field that you make more than 1 registration for you school.

As you can read there, we charge organisation costs beside the course fee and we give discount if you register 5 or more participants for Euneos courses in the same school year. We don't discuss our prices and we don't make exceptions.

It should be the teacher or staff member who - on behalf of all participants of the school - takes care of correspondence with Euneos about administration issues: billing, change of names of participants, adding or cancelling participants. This person is responsible for delivering all information to all participants of his/her school. Shortly before a course Euneos will also send information directly to the participants. IMPORTANT: only the contact person will receive confirmation of the registration.(Required.)

We will need the names of the teachers for the list of participants, for names on bills and for certificates.
IMPORTANT: fill in names as they should be written on the certificate. Please do not fill in the names in capitals. Only capital for the first letter of each part of the name.
