Introduction
Thank you for taking the time to complete this survey for the Town of Reading’s Community Health Needs Assessment. This assessment is being done by the Reading Public Health Department, in partnership with the Board of Health, other Town Departments, and local health and human services providers. Your answers to these questions will help us identify community needs and solutions. All responses will be kept anonymous. Responses will only be shared in combination with other completed surveys. Individual responses will not be shared.
There are four sections of this survey, and it will take about 10 minutes total to complete all four sections. At the end of the survey, there will be more information about an optional raffle drawing for a gift card.
