Please read the following instructions very carefully.

In the first part of the assessment, you will be asked to rate yourself 1-10 (1 being low, 10 being high) on a number of statements. This will be followed by a series of open-ended questions designed to get you thinking deeply about your role within your team. Finally, you will be asked to provide some basic information about yourself.

As you answer each question, please do so carefully and thoughtfully. Be as honest as possible, and evaluate each statement based on where you and your team are right now (instead of where you hope to be one day). This will help you create an accurate starting point for significant transformation.

Finally, although we highly recommend completing the assessment in one session, it is possible to begin the survey and complete it at a later time. However, you must be using the same computer and internet browser in order to accomplish this. If you accidentally exit the survey prematurely, you can resume where you left off using the link you were provided.
