Thank you for your interest in the P4P Supports Directory for Independent Facilitators and Planners. This online directory will assist families in finding trusted Independent Facilitators and Planners, vetted by P4P.

For the purpose of this application we would classify a facilitator as a person who is not employed or funded by an organization that provides direct services or funding. A planner is a person who provides individualized support to an individual culminating in a completed plan (housing, employment, future).
