There are 2 pages to this screening - BOTH pages must be completed to be contacted regarding participation. You are currently on page 1.

Question Title

* 1. Please provide your contact information:

Omneuron, www.omneuron.com, is a neuroscientific clinical research company located in Menlo Park, CA. We use MRI and EEG imaging technology to study brain activation in different mood and disease states. There is very low risk to participating in either of these procedures, however, if you have any questions about the technology or study, please contact Ella Laramee at 650-585- 5304 or research@omneuron.com.
All participants must be able to attend 3 or more visits to our Menlo Park, CA office. We are accessible by car from 101 or 280, Cal Train or SamTrans.
All participants are compensated for their travel time to and from our facility at $10 an hour, completing paperwork at $10 an hour, time spent in the MRI/EEG at $25 an hour and any applied pain sessions at $30 an hour.

Question Title

* 2. Purpose of Study:
The goal of this neuroimaging study is to apply functional Magnetic Resonance Imaging (fMRI) to study brain activity of healthy people in comparison to those with chronic pain conditions. We are comparing healthy individuals with chronic pain patients to detect consistent patterns of brain activity for diagnostic development and novel new treatment modalities.
You may be asked to participate in some or all of the following types of visits:

Medical Exam: Our onsite physician will perform a routine physical and health history
Strategy: In this section, you will be trained in mental tasks and then use them while you undergo MRIs and/ or eegs. You may or may not receive real-time fMRI or EEG feedback of your own brain activation. You will see the activation in the form of a virtual reality interface.

Resting State: In these visits you will undergo up to 3 MRIs and/or up to 3 eegs with no task (resting).

Applied Pressure: In this section, you will undergo several MRIs and/or eegs and receive a varying pressure pain stimulus on your thumb both in and out of the scanner. This pressure is applied by a small plastic disc for about 4 seconds. Your ratings will determine the range within which the pressure will be applied, starting very mildly and gradually increasing. You decide your level of participation.

These studies allow us to learn if you can learn greater control of specific regions of the brain that are related to your pain experience. We'll then see if there are any corresponding changes in the pain you feel.

Question Title

* 3. Let me first give you a list of reasons that would exclude you from participation:
• you are younger than 21 or older than 65,
• you have metal piercings that can't be removed, surgical clips, a pacemaker, or other implants that cannot be removed and are not MRI compatible;
• you have a tattoo on or near your face;
• you have a history of claustrophobia;
• if you weigh over 220 lbs AND are shorter than about 6' - the height/weight requirement has to do with fitting in the scanner.
• you are pregnant or expecting to become pregnant; you have any metal fragments in your eyes,
• you have or have had within the past year severe psychiatric illness,
• you have a condition that renders you more sensitive to noise than usual,
• or if you have extensive dental work.

Do any of these criteria apply to you?
