Thank you for participating in this survey from the President's Committee on Child Care Options. We seek your feedback as part of our initial efforts to assess data, attitudes and opportunities three years after the first President's Committee on Child Care Options completed its work.

This form is anonymous and you may choose to answer as many or as few questions as you like. You will note that specific demographic information, including income, is requested. This quantitative information will help the committee better understand details about our community.

As the academic year proceeds, the committee will continue to report back to the community regarding findings and recommendations. We may even issue another survey to gather additional insights from all of you.
