Background:
Welcome! You are invited to participate in a research study on “Evaluating Data and Voice Signals in Pathway Survivable Cables for Life Safety Systems” conducted by the Fire Protection Research Foundation (FPRF), the research affiliate of National Fire Protection Association (NFPA), where the objective is to determine if temperature impacts the transmission and the functional and operational quality of alarm/data signals and voice messages in fire and non-fire rated environment. You can read more about the overall project from this project summary document.

This questionnaire has been created as part of this research study to gain insight from you, and others you may know, to identify incidents of operational cable failures for life safety systems due to fire or effects from fire and, if possible, indicate if elevated temperature played a role in the failure. This information will assist the NFPA 1225 Public Safety Telecommunicator Professional Qualifications Technical Committee in the development of the next edition of NFPA 1225, Standard for Emergency Services Communications.

Confidentiality: Your participation in this research survey is voluntary. You will receive no payment for participating in this study. You may skip any questions that make you feel uncomfortable and can stop the survey at any time. The information you provide through this survey will be anonymous. This means that your name or your organization’s name will not be collected or linked to the data in any way. The researchers will not be able to remove your data from the dataset once your participation is complete. The anonymized and compiled results of this survey will be used in the final report, the researchers will not identify you or your organization. The final report will be published publicly from the Fire Protection Research Foundation website (www.nfpa.org/foundation).

If you agree to participate in this study, please complete the survey below. It is estimated that the questionnaire will take approximately 2 minutes to complete.

Question Title

* 1. Are you aware of any operational cable (defined as any conductor, optical fiber, radio carrier, or other means for transmitting system information to remain operational during fire conditions) for life safety systems that failed due to fire/fire effects?
