This survey is approximately 10 pages long and should take 10-15 minutes to complete.
Several questions in this survey have been adapted from a publicly available survey from the National Survey of Children's Health, using the survey questions from the 2003, 2007 and 2011/2012 data. The details of the NSCH, questions adapted and the actual data, can be found at this website:http://childhealthdata.org/browse/survey
A few important notes before you start:
-Once you start the survey, please complete it because your responses will not be saved if you leave the website.
-An asterisk * indicates that the question must be answered to continue with the survey.
-The purpose of this study is to explore the association between parenting styles and the relationship between the parent and the child with ASD. The information gained from this survey may determine which parenting styles are most associated with a strong, positive relationship between parent and child on the Autism Spectrum.
-All questions will be referring to your eldest child with ASD unless otherwise specified.
-Throughout this survey, the term ASD will be used to refer to Autism, PDD-NOS, Asperger's Syndrome, Rett Syndrome, and Childhood Disintegrative Disorder
-Your responses will be kept completely confidential. If you would like to know the group-level results of this survey, please send a separate email to kevin.carroll@yorktown.org with "Autism Survey Results" as the subject. I will be happy to share my results with anyone who would like to know. (As the survey is anonymous, result requests written in the comment box will not be answered.)
