Survey Information Sheet and Consent Questions

Thank you for participating in this study.

This survey is about 19 pages long and should take about 15 to 20 minutes to complete.

A few important notes before you start:
- Once you start the survey, please complete it because your responses will not be saved if you leave the website.

- An asterisk (*) indicates that the question must be answered to continue with the survey.

- The purpose of this study is to explore the association between implementing autism treatments at home for children with ASD after receiving training compared to the implementation of autism treatments at home without receiving training. The information gained from this survey may determine gaps in the delivery of autism treatments, which is important in understanding the effectiveness of treatments for children on the autism spectrum.

- All questions will be referring to your eldest child (including twins) with ASD unless stated otherwise.

- Throughout the survey, the term ASD will be used to refer to autism, autism spectrum disorder, Pervasive Developmental Disorder-Not Otherwise Specified (PDD-NOS), and Asperger's Syndrome.

- Your responses will be kept completely confidential. If you would like to know the group-level results of this survey, please send a separate email to debralee.dobransky@yorktown.org with "Send Results Please" as the subject. I will be happy to share my results with anyone who would like to know. (Since the survey is anonymous, result requests written in the comment box will not be answered.)
