As a result of the Colorado Opioids Settlement, Region 4 has been granted funding to support five different areas of Opioid and Substance Use-related support—the settlement MOU specifies that funds can only be used for state-approved purposes related to opioid abatement. The approved purposes include a list of strategies spanning Prevention and Education, Harm Reduction, Treatment, Recovery, and Criminal Justice.

OMNI Institute (OMNI) is collecting information from individuals on the Region 4 Opioid Council, associated Advisory Committee, other vested community partners, and community members with lived/living experience to ensure that these funds are allocated equitably and based on the needs of Region 4. This survey aims to identify funding priorities that align with the approved purposes and build on a resource scan to identify substance use supports within the region. The results of this survey will support shared decision-making around future RFP development and funding allocations.

This survey should take approximately 15-30 minutes to complete. If you have any questions, don't hesitate to contact OMNI at Thank you for participating in this survey.
