Thank you for your interest in attending the "One City, Many Voices Engagement Summit" on Monday, February 5 from 5:30- 8:30 p.m. at The Art Gallery of Greater Victoria.  We look forward to having you join us.  Please complete the following questions, so we can do our best to host you.

* 2. Please provide your email address.  (It will only be used to confirm your registration for this event and to send any follow up materials specifically related to the Engagement Summit.)

* 3. If you would like to register additional participants for the Engagement Summit, please indicate the number of participants and include their names below:

* 4. We will provide a light dinner.  Do you have any dietary restrictions?  Please let us know if you are vegetarian, vegan or have allergies such as gluten, etc. (If you have registered for more than one person, please include the number of participants for each dietary request.)

* 5. Please let us know if you need childcare for this event.

* 6. If your children are between the ages 4- 12 they can be registered for an on-site Studio Art Class with Art Gallery staff.  There are limited spaces in this program.  Please indicate the number of children ages 4-12 that you would like to register in this program.

* 7. If you have children younger than 4 that need childcare, please include their ages and the number of children that need care.  There will be a limited number of spaces for childcare. We will be in touch to confirm what we can offer.
