Thank you for being part of the OLPH community! We love working together with you so that we can acknowledge and recognize what is going well, and identify areas of improvements. Community Feedback is monitored and reviewed by members of the OLPH School Council and discussed during monthly meetings. Community feedback is anonymous. If your entry requires a direct reply please utilize the handbook, or the Who to Consult page, so that you can bring your comments or suggestions to the correct party directly and receive a reply. Community feedback is meant to improve our school and build up our community. Thank you for partnering with us in our mission to deliver academic excellence in a Catholic faith-based community. 
1.I acknowledge that this is not the correct avenue for concerns or feedback regarding my child’s courses, their teachers, or other personnel at OLPH and that the correct protocol for these concerns can be found in Section X of the OLPH handbook entitled home/school cooperation under Section A. Communication.(Required.)
2.I understand that if I choose to use this modality to express my concerns about a particular course, teacher, or school personnel my response will not be addressed during council meetings or in the school newsletter.(Required.)
