Welcome to the Service Scorecard! October's theme is Off the Court.
This month, we encouraged participants to complete at least five acts of service, like a series of five free throws or "shots", that center on sports, staying active, and engaging with their communities.* All participants that finish a set of shots and complete and submit this form will receive Hoop Shoot swag and be entered in a drawing to win a $100 donation to the charity of their choice. Participants that complete less than five projects will still be entered into a drawing for Hoop Shoot swag. Don't forget to share your service projects on social media using #HoopShootServes!
