Please complete this report immediately after each rotation. You must submit all three rotation reports before you’ll be permitted to take the qualifying exam. The reports are kept in your file and reviewed by your qualifying exam committee prior to taking the exam. Your rotation PI will also submit an evaluation of your rotation and these will be reviewed as well.
