You are invited to take part in a New Mexico Office of African American Affairs research survey about the health and wellbeing of mothers and their infants in New Mexico. This survey will take approximately 20-25 minutes. YOU MUST BE 18 YEARS OLD or OLDER TO TAKE PART IN THIS STUDY. We are interested in reducing negative birth outcomes, particularly the serious disparity seen among infants born to Black/African American mothers. All survey questions are important, so please answer ALL questions in the survey that apply. We are working hard to understand what’s working and what is not for women in New Mexico during our pregnancies and postpartum period. Thank you for helping us with this effort. Some of the questions asked may cause discomfort but there are no risks in taking the survey. Taking part in this survey is COMPLETELY VOLUNTARY and your answers will remain ANONYMOUS. If you have any questions about the survey, how it is conducted or want a copy of any results published, please contact Raquel Z. Rivera at firstname.lastname@example.org, Prof. Rongal Nikora at email@example.com or Sunshine Muse at firstname.lastname@example.org. Special thanks to New Mexico-based African-American midwife Nandi Hill for her input and support in the development of this survey.
