Please complete the following information thoroughly. The information provided will used to support your eligibility for the Nia Trainer Training program. This application is required to attend the 3-day Nia Trainer Event: Courting Your Destiny August 12-14, 2010.

This form must be completed in one session. Before you begin, we encourage you to look through the questions on each page. This will help you get a sense for the kind of information and responses you’ll need to provide.
