REASONS WHY
This survey aims to help NF Midwest gain insight into the NF community's needs and help us better communicate. The survey will take about 1 to 20 minutes. Your responses and data will not be shared. You can read more about our privacy policy on our website www.nfmidwest.org/privacy.
THREE PARTICIPATION SECTIONS
SECTION ONE - UNSUBSCRIBE - In the first part of this survey, you may "unsubscribe" from all communication with NF Midwest. Once processed, you will be removed from emails and regular mail. You must provide your contact information so we can find and remove you.
SECTION TWO - GENERAL COMMUNICATION INTERESTS - If you continue with this section, you will share your communication preferences and help us learn more about how you get your news. You will also be asked about general interests such as walks, research news, etc. This data will help us learn more about our community to improve communications and a little about you specifically. We will then do our best to tailor what you receive.
SECTION THREE - WAYS NF AFFECTS YOU - If you choose to participate in section three, you will be asked more specific questions. You can choose whether or not to answer a question. Participating in this section will teach us much more about you and your household and how NF affects you or your family. Participating in this section will better enable us to notify you in more specific ways. For instance, if you indicate a plexiform tumor, we may work more actively to ensure you know about any trials or new information on plexiform tumors. The information in this section will also provide more demographic information. The demographic information and information on how NF affects you will help us with our programming. It may also help us in getting sponsorships or grants. The specific information you give will never be shared with anyone else, including your identity.
