Connections: Community-Transport-Health-Initiative

Lancefield and Romsey Neighbourhood Houses have been funded to research transport needs in the community.

We invite you to share your thoughts and experiences to enable us to determine the transport needs of our community.

The survey is designed in two sections, medical and social. 

Please answer questions taking into account all members of your household.

* 3. How many people live in your household per age group (excluding yourself)?

* 4. Whilst living in the district of Romsey and Lancefield, have you ever needed to travel outside of the district for medical appointments?

* 5. If you answered yes, to question 4, please tick what kind of transport you have used. If you answered no, please go to question 6.

* 6. Have you ever delayed making or postponed a medical appointment due to lack of available transport?

* 7. If yes, this was because? (tick as many as applicable)

* 8. What suburb/ suburbs was this appointment in?

* 9. What method of transport do you use to get to local appointments?

* 10. Have you ever been to a medical appointment and then been unable to get home due to transport issues?
