Title: North Carolina PMP Economic Survey
Investigators: Ashlynn Spell, Mike Waldvogel, and John Russ - NC State University

The  NCSU Departments of Entomology & Plant Pathology in partnership with the North Carolina Pest Management Association is conducting this economic survey of pest management professionals working in our state.  Ms. Ashlynn Spell is a Junior  majoring in Agricultural Business Management.  She is using this project as part of her undergraduate degree program.  Dr. Mike Waldvogel is Extension Associate Professor for Structural and Industrial Pests.  Dr. John Russ is the Undergraduate Program Coordinator in the Department of Agricultural and Resource Economics.

We ask if you could complete this survey before Friday, May 12th.

What are some general things you should know about research studies?
We are asking you to take part in our research study. Your participation in this study is strictly voluntary.  You have the right to be a part of this study, to choose not to participate or to stop participating at any time without penalty. The purpose of research studies is to gain a better understanding of the pest management industry. You are not guaranteed any personal benefits from being in a study. Research studies also may pose risks to those that participate. In this consent form you will find specific details about the research in which you are being asked to participate. If you do not understand something in this form it is your right to ask the researcher for clarification or more information. A copy of this consent form will be provided to you. If at any time you have questions about your participation, do not hesitate to contact the researcher(s) named above.

What is the purpose of this study?
The goal is to assess the economic importance of the pest management industry in North Carolina and to examine industry trends across the state.

What will happen if you take part in the study?
If you agree to participate in this study, you will be asked to complete the following survey. We estimate the completion time to be 15-20 minutes.

Risks
There are no risks associated with this survey.

Benefits
The benefit of this survey will be to provide you with information about  key economic factors affecting  pest management professionals in North Carolina.

Confidentiality
Although the data collected in this survey are a matter of public record.  The published summaries of the data will not identify individual respondents.

Compensation
You will not receive any direct payment for participating. However, participants completing the survey will be included in a drawing for one free registration to attend the NCPMA's 2018 Pest Control Technician School - January .22-25.
