Mary Our Queen Catholic School Application Process
Listed below are the required documents for enrollment.

Student Information/Emergency Contact Form (1 per family)
Archdiocese General Information Form (1 per student)
Student Health Form (1 per student)
Dismissal Instructions (1 per family)
Family Handbook and Technology Use Policy Agreement (1 per family)
Authorization for Release of Records if student is transferring from another school
Copy of each student’s birth certificate
Copy of each student’s baptismal certificate (if applicable)
Copy of each student’s immunization record
$250 New Family Registration Fee (This fee is non-refundable. Once enrolled, this fee is applied to the 2021-22 tuition.)

Once enrolled, families will receive an email from FACTS Management Company with information to set up their tuition payment account.
