By submitting this form, I am committing to a sponsorship for the Martini Masquerade and understand that sponsorships are nonrefundable.
Payment must be received by March 18. Sponsorships not paid by this date may be released.
The Martini Masquerade is an indoor event and is expected to take place as scheduled. If the event is postponed, disrupted, or canceled due to circumstances beyond our control, including severe weather, public safety concerns, travel advisories, or public health emergencies, sponsorship funds will not be refunded and may be applied as a monetary donation to Companions for Children or transferred to the next Martini Masquerade fundraiser.
