Introduction Page/ Confidentiality Consent

Introduction:  
Hello and thank for your participation! My name is Cindy L. Arenas Nieves. I am a doctoral student, at Northcentral University. I am conducting a research study on what is the experience of military youths when their parent deploys. I am completing this research as part of my doctoral degree. I invite to complete the survey.

Activities: 

If you participate in this research, you will be asked to:
1.      Complete a web-based survey and describe your experience.

Eligibility: 

You are eligible to participate in this research if you:
1.      You are between the ages of 18 and 22.
2.      You have a parent who is or has deployed within the past 5 years.
3.      You have a civilian parent with whom you stayed during your parents’ deployment.
4.      You are an American Citizen.
You are not eligible to participate in this research if you:
1.      Your parents are dual military.
2.      Your parent has not deployed during the last 5 years.
3.      You have already completed this survey.
4.      You are not an American citizen.

I hope to survey 12 to 36 people in this research project.

Risks: 

There are minimal risks in this study.  Some possible risks include: re-experiencing distress, feeling sad, or uncomfortable. To decrease the impact of these risks, you can: stop participation at any time.

Benefits
:
There are no direct benefits to participating in the survey.

The potential benefits to others are that your views will help to inform people who are trying to help you and other like you through the experience of parental deployment.

Compensation:

To thank you for your willingness to participate, you will be given $10 Amazon EGift Card for completing the survey. This will require that you input your email address in the comments section at the end but will not be linked to your responses.

Confidentiality: 
The information you provide will be kept confidential to the extent allowable by law. Some steps I will take to keep your identity confidential are: the survey procedures will be within anonymous data collection. This means that no IP addresses will be collected during your visit.
The people who will have access to your survey answers are: myself, my dissertation chair, and my dissertation committee. The Institutional Review Board may also review my research. None of us will be able to know any personal identifiable information.
I will secure your information with these steps: a) I will download the digital copy to my computer and password protect the file. b) I will further make a copy of the file on a thumb drive which I will also password protect. C) While I have an account with SurveyMonkey (1 year or after all data is collected, whichever is longest), the data I collect will be secured as dictated by SurveyMOnkey’s Survey Rules of Privacy.
I will keep your data for 7 years stored in password protected files and thumb drive. Then, I will delete all electronic data.
