The purpose of this survey is to record your experiences with Middlesex County facilities, programs and services as we create Middlesex County's Joint Multi Year Accessibility Plan, partnering with The County’s local municipal partners.

The feedback provided will help inform the County about gaps and barriers that currently exist for people with disabilities while becoming better equipped to reduce or eliminate future barriers in our communities.

Accessible formats and communication supports are available for this survey in an alternate format.  Please contact the County’s Accessibility Coordinator at 519-631-1460 ext. 167.
