Tobacco Impact Assessment

As a part of our Missouri Foundation for Health (MFH) grant, we would like to take this opportunity to learn about tobacco usage in the county. We ask that you would take a few minutes of your time to complete this questionnaire. The questionnaire is voluntary and anonymous. No names or other identifiers will be collected. All information obtained from this questionnaire will be kept confidential and an aggregate report will serve as a tool to improve the health of the community. There are no wrong answers.
