NDSU Extension Service - Department of Health, Nutrition and Exercise Sciences; EML 316; North Dakota State University

Men are invited to participate in a short online survey about nutrition and health being conducted by Julie Garden-Robinson, NDSU Extension Service Food and Nutrition Specialist, Cindy Klapperich, Extension Agent, and Kristi Berdal, Extension Agent. If you are a woman and would like to fill this out based on your perceptions of men, you can do that, too. However, please be sure to mark the "female" box later in the survey. If you are filling out the survey for a man and your answers are based on his responses, mark "male."

The NDSU Extension Service Nutrition and Health program is conducting a research study to learn which nutrition/health topics are of interest to men ages 18 and older and how men prefer to receive health-related information. Your participation is entirely voluntary, and you may withdraw from participation at any time. Participating in the survey will take about 10 minutes of your time. The survey is anonymous, and we foresee no risks related to participating in the survey. All data will be presented in summarized form. We will use the information to create health education materials for men. We will have prize drawings for items such as two $50 gift cards, calendars, cookbooks, as a small token of our appreciation for your assistance. Upon completion, you will be taken to a separate web page to type your name, address and email so we can mail the prize and alert you if you won.

For questions about the rights of research participants, or to report a problem, please contact the NDSU Institutional Review Board (IRB) at 701-231-8995 or tolle-free at 1-855-800-6717 (ndsu.irb@ndsu.edu) or Julie Garden-Robinson at 701-231-7187 (e-mail: julie.garden-robinson@ndsu.edu).

We greatly appreciate your input. Thank you for being part of this project!
