Disaster Preparedness Area of Interests Survey: Practice-Based Providers/Medical Homes

The American Academy of Pediatrics and the Pediatric Pandemic Network have developed this survey to gain insight into the needs of practice-based/medical home facilities to enhance their pediatric disaster readiness. The findings will inform the development of educational initiatives for an upcoming pilot program. The survey is three questions and should not take more than a few minutes.
