All personnel who are officiating or working in a support services role at The ASRL Open 2016 are required complete the following medical questions.
This medical form is part of the ASRL's duty of care and will not be used for any other purpose other than having some medical and health information readily available on site if an incident or accident was to occur. We appreciate that some fields in this medical form duplicate fields on the nomination form.
However The ASRL is required to collect this personal medical data separately to your officiating nomination.
