This form must be completed by an adult who will become a MacqCon member and be responsible for all fee payments.

A separate Student Information Form is required for each student enrolling in 2026, including both new and returning students.
Families enrolling multiple students can use the same link to start a new form for each.

Before completing the form, please review the MacqCon Member & Student Handbook, which includes details on lesson fees and student guidelines.

After assessing each Student Information Form, MacqCon will contact you with available lesson options. If a lesson time is offered, the enrolment is for the entire calendar year.
Cancellations must adhere to the guidelines in the MacqCon Member & Student Handbook.

Privacy Statement: All information provided will be used solely for enrolment, enquiry, and event notification purposes. It will remain private and will not be shared with third parties without consent, except where legally required.

As a condition of funding from the NSW government, MacqCon is required to collect demographic information for statistical purposes. Individual details will not be shared with the NSW government or other parties.

Providing information on medical conditions, special needs, or learning requirements will help our staff provide the best possible tuition and care. Any information shared with the NSW government will not include student names or contact details.
