Thank you for participating in MAPPING MUSIC. This survey gives you a chance to tell about current MUSIC PROGRAMS at your school, and to share a WISH LIST of any RESOURCES NEEDED to strengthen those programs or initiate new ones. Even if your school does not currently have music programs, please complete the survey so we can share your music wish list.
Before you get started, be sure you have the following information for each type of music course or program at your school. Please collaborate with other teachers as needed to complete the form:
