This Whānau Submission form is for the Māori inquiry into Oranga Tamariki. The inquiry is by Māori, for Māori, with Māori and is an independent review. The Whānau Submission is completely voluntary, and you can choose not to respond to any particular questions.

By responding to this survey, you give consent to your information being used by the Māori Inquiry into Oranga Tamariki.

Any information you provide will be confidential (i.e. your name will not be used) and will be used to inform the final inquiry report. Online submissions close October 31st, 2019.
