LifeBridge Participant Registration

The information that you enter below will help us plan LifeBridge activities that will help meet your needs. Your personal data will only be used for the purposes of meeting your needs with LifeBridge programmes and will not be disclosed to anyone without your personal consent. LifeBridge is a non-profit volunteer organisation incorporated in Perth, Western Australia. By completing this information you consent to be invited to participate in LifeBridge programmes. We will ask for further information from you, and we will evaluate your credentials before confirming that you can participate. The number of participants is strictly limited so we apologise in advance if we cannot accommodate your participation immediately. Participants will be asked to pay a nominal fee to cover administrative costs and educational materials.
