Hello. My name is Anna Morrison, MS. and I am a doctoral student at Philadelphia College of Osteopathic Medicine. I am asking you to fill out three forms about a study I am doing. I am looking at people’s beliefs about psychotherapy and I am working with Robert A. DiTomasso, PhD, ABPP, professor of psychology. If you are part of this study, you will be asked to fill out three brief forms, which will take about 15 minutes of your time. No one will be able to find out who you are or connect you to your answers. Becoming part of this study is voluntary, and you may stop at any time for any reason without any penalty. Your answers may help us to better understand the beliefs people hold. By filling out these forms, it is possible that in some people it may cause mild discomfort.
