The Los Angeles Department of Mental Health (DMH), Los Angeles County Office of Education (LACOE), and the University of California at Los Angeles (UCLA) have partnered to evaluate a strategy to organize school and community resources. We are asking all students from this school to complete this survey to help us improve the quality and impact of this strategy for school staff, parents/caregivers, and students. This survey should take approximately 15 minutes to complete and will help to create a better learning environment for everyone at the school.

Participation in this survey is completely voluntary. There are no penalties for declining to participate in the survey or for declining to respond to items on the survey by selecting "N/A" or "Decline to respond." Your survey response will not have any impact on your school records or family's eligibility for services and support.
