This survey is for all Kentucky public libraries that offered a Summer Reading Program (SRP) in summer 2016 regardless of the theme used or ages served. Your responses will help the State Library, partnering state agencies, and the Collaborative Summer Library Program to provide helpful resources and support for future summer reading programs. Your responses will also help demonstrate the importance of libraries as leaders in summer enrichment, literacy support, and workforce development. Instructions: only one person from your library system should complete the survey. The survey must be completed in one sitting. If you have all relevant statistics and anecdotes before the start of the survey, completion should take approximately 20 minutes.The deadline for evaluation submissions is Friday, September 30, 2016.

* 2. Libraries offer Summer Reading Programs for different reasons. What is the primary goal, or main purpose, of your library's SRP? Think like Twitter and keep your answer to 140 characters or less.

The survey will address services and programs to library patrons identified by 4 broad categories: Young Children (identified as birth-preschool traditionally served through early literacy efforts, also known as "pre-readers"), Older Children (identified as school age, entering kindergarten and rising), Teens (identified as those entering 6th grade and rising), and Adults (identified as 18 or older and high school graduates that have aged out of teen programming). If such information is not self-identified for all patrons served (for example, homeschool students), please provide your best estimate.
