Survey Deadline - Monday, October 1, 2018

This survey is for all Kentucky public libraries that offered a Summer Reading Program (SRP) or Summer Learning Program (SLP) in summer 2018 regardless of the theme used or ages served. Your responses will help KDLA, partnering state agencies, and the Collaborative Summer Library Program to provide helpful resources and support for future summer learning efforts. Your responses will also help demonstrate the importance of libraries as leaders in summer enrichment, literacy support, and workforce development. We know your time is limited and valuable. As a thank you for participating in this voluntary survey, KDLA will provide your library with one (1) digital copy, via USB flash drive, of the upcoming 2019 Collaborative Summer Library Program (CSLP) Summer Reading Program Manual. Manuals are expected to arrive and will be distributed through conferences and continuing education opportunities later this fall.

Instructions: only one person from your library system should complete the survey. The survey must be completed in one sitting. If you have all relevant statistics and anecdotes before the start of the survey, completion should take approximately 20 minutes.
The deadline to submit an evaluation is Monday, October 1, 2018.

The survey will address services and programs to library patrons identified by 4 broad categories: Young Children (identified as birth-preschool traditionally served through early literacy efforts, also known as "pre-readers"), Older Children (identified as school age, entering kindergarten and rising), Teens (identified as those entering 6th grade and rising), and Adults (identified as 18 or older and high school graduates that have aged out of teen programming). If such information is not self-identified for all patrons served (for example, homeschool students), please provide your best estimate.

The next three questions ask you to report on the number of people who participated in your SRP and related programming. Please provide the numbers of people who REGISTERED for your SRP, who ATTENDED SRP-RELATED PROGRAMS, and who COMPLETED your SRP. If you count SRP participation by some method other than registration, skip ahead to Question #7 and describe your method and results.
