1. A Survey of Persons with Surgically Confirmed TE/DE

The purpose of this research is to gather histories of a of medical encounters, resource use and practices as reported by a select population of persons with surgical confirmed thoracic and/or diaphragm endometriosis.  

You must be 18 years old.  Participation is voluntary, without compensation and anonymous.  No personal identifying information, or IP address of your electronic device is collected. There are no foreseeable risks with participation. You are not obligated to complete the survey and can discontinue at any time.   

The survey should take less than 15 minutes to complete. Based upon your experiences, you will be asked to answer between 1 and 28 questions.

The principal investigator is Wendy Bingham DPT, Executive Director of Extrapelvic Not Rare. Extrapelvic Not Rare is a 501 c3 charitable organization.  This study is not sponsored. The principal investigator has no conflicts of interest to disclose.
