アンケートへのご協力のお願い （同意の確認）

この度は、アンケートにご協力いただきありがとうございます。本アンケートは、8号館の施設とサービスに関する学生の皆さんの意見収集を目的に、ELIとSALCの教員で構成された「KUIS 8研究チーム」によって作成されました。
本アンケートへの参加は任意で、いつでも中断することができます。
このアンケートは、学生の皆さんが現在KUIS 8をどのように利用しているかを把握し、今後の8号館運営上の意思決定に役立てることを目的としています。また、アンケート結果を学内研究会、専門学会、学術誌等の研究発表に利用いたします。このアンケート結果から個人が特定されることはありません。
アンケートの所要時間は約15分です。8月10日までにご回答ください。なお、アンケートにご協力いただいた学生の中から抽選で5名の方に粗品を差し上げます。

Thank you for participating in the survey. This survey was designed by the KUIS 8 Team in order to get students' input about the services and facilities. Participation in the survey is completely voluntary, and you may choose to stop at any time.
The survey results will help us to understand how students are using KUIS 8 and will be used to improve KUIS 8 facilities in the future. The research findings may be used in internal seminars and reports, at conferences, and in academic journals. The survey results are anonymous and any information that personally identifies participants will not be included.
The survey will take about 15 minutes to complete. It will be closed on 10th August. Those who complete the survey will be eligible to enter a lottery to win one of 5 small gifts.
