Thank you for participating in our survey. Your feedback is important for planing future meetings.  Each session and speaker must be evaluated by those that attended the session in order to ensure full credit.
 
1) CPE Monitoring credit - You need to provide your information in order to get credit for the sessions you attended.
 
2) Even though you need to provide your name as part of the survey to get CEPE credit, the results will remain anonymous for all questions asked.
