UCEAP Tokyo Study Center - Final Checklist

You are finishing up your program soon. Before you leave Japan, please make sure to complete this final checklist and take care of the things listed below. Thank you for your understanding and cooperation in advance.

3.Post-EAP E-mail Address (preferably an email you can use even after graduating from UC)

We might need to contact you after your departure regarding undelivered mails, UC registration, etc. Please provide your Post-EAP address.
4.Things to complete before you leave Japan

Submit all the documents required by your host university.
Returned all the items (e.g. student ID, library books, library card, keys, swipe cards etc.) that your host university / housing requires you to return.
Complete Moving-out procedures (転出届 tenshutsu todoke) and terminate your National Health Insurance at your city office.
Discontinue utility services/cell phone/land line/internet provider/cable T.V. in Japan and pay the balance.
Pay any other outstanding bills / fees in Japan if any (e.g. unpaid library fines).
Close your bank account(s) in Japan.
