We would like your views on community health services
Community health services are not provided in a hospital setting and instead take place in local clinics, patients’ homes or residential and nursing homes etc.
Some examples of community health services include:
• district nurses and community matrons
• podiatry
• dieticians
• physiotherapists
• occupational therapists
• treatment room services
• leg ulcer clinic
• smoking cessation clinics
• continence service
• end of life care teams

Your local NHS in West Lancashire has a new vision for health services to better support its local communities. We are trying to improve our services and ensure that both health and social care services work better together in a more joined up and coordinated way. As part of this move towards joined up care, we would like to hear from West Lancashire residents about their experiences of using community health services and social care services. Your views will be considered and used to inform our re-procurement of the local contract for community health services. The procurement also includes some aspects of urgent care such as GP out of hours, walk-in centres and the Acute Visiting Service.
We need to know what is working well with the services you receive now and also what parts you feel could be improved. We’d like to hear from everyone, but especially if you are, or someone you care for is, over 65 years old and/or has two or more long term conditions e.g. diabetes, breathing problems, heart problems, we would welcome your views on the suggested improvements.
