Welcome to the Jewish community of Victoria Disability Survey.

The Jewish community has an important tradition of supporting its members living with a disability to lead fulfilling and positive lives. In an ever-changing landscape we need to provide services that meet the needs of the community now and into the future. The NDIS is a great opportunity for people with a disability to get the services they need to be able to maximise the potential in their lives and we are keen to help you take advantage of that.

This survey is extremely important. Your answers will help us decide where to best use our resources and what government funded supports we should apply for that will most affect your life or the life of the person you support.
 

You can...
  • answer these questions for yourself,
  • get someone to help you, or
  • fill it in on behalf of a person with a disability.
 
 

 
If you prefer a telephone version of the survey, you can:
  • call a staff member of Jewish Care Victoria on (03) 8517 5934 Monday or Tuesday between 10 am and 2 pm, or
  • email your contact details to surveys@jewishcare.org.au if you want us to call you.

If you are filling this out for someone else, please fill in the information as it relates to the person.

 
The survey takes about 15 minutes to complete. 

At the end of the survey you will have the opportunity to be contacted to take part in a NDIS information session held early next year. During this session you will be able to discuss these matters with Jewish disability service providers in Victoria directly.
 
 
   The survey is completely anonymous.


    Your data will remain private and is only visible to the research team as anonymous data.
