Prevention Strategies to Protect your Bottom Line is a key focus of DFWBGH this year.  We are conducting a series of Employer Surveys during the summer to assess current prevention strategies, including vaccinations, cancer screenings, and chronic conditions management.

We would greatly appreciate your participation in DFWBGH Prevention Survey#1: Role of Vaccinations.  The survey should take approximately 5 minutes to complete.  Your responses will be kept confidential and reported only in aggregate.

In appreciation for your thoughtful and timely responses to this Survey, your name will be entered in a drawing for a $100 gift card.  Five winners will be drawn, so the odds of winning are very good!
Survey Deadline:  July 31, 2023

Note: Watch for Prevention Survey #2  on the Role of Cancer Screenings
