Hello and Welcome to the IGYM Fit|Life Preliminary Application. This questionnaire was designed for us to get a clear and accurate snapshot of your health and wellness goals. You will notice that our questions do not only cover physicality but also mentality. This is completely intentional. The IGYM Fit|Life Coaching program was designed to guarantee growth in both areas of our client's lives. We define true health as being sound in body and mind. This survey covers both bases.
