Brain Injury Identity Card Survey 2025

This short survey is for anyone who has a Headway Brain Injury Identity Card, or anyone who uses a smartphone in everyday life.

Your answers will help us to understand how the ID Card is working and whether there’s a need for a smartphone app version. We would also like to find out if there is a need to develop a digital assistant to support people living with brain injury.
