Information and Consent

Thank you for your interest in our survey!

Drs. Alvin Simms and Atanu Sarkar from Memorial University are conducting a study on the utilization of the YMCA recreational facility in Marystown. This project is funded by the YMCA and as a member of the YMCA facility the researchers are interested in hearing your opinions about the facility and about your use of the facility in Marystown. 

Taking part in this study involves completing an online survey that will take approximately 15 to 30 minutes of your time to complete. Your participation in this research study is completely voluntary. You may choose not to participate. If you decide to participate, you may withdraw at any time and you are free to leave out any question you do not wish to answer. If you choose not to take part in this research or if you decide to withdraw from the research once it has started, there will be no negative consequences for you, now or in the future.

We will follow all protocols to keep your information confidential. Your responses will be confidential and we do not collect any identifying information such as your name, email address or IP address.  All data is stored in a password and encrypted protected electronic format. The results of this study will be used for report purposes only and the “raw data” will not be shared with YMCA representatives.

The data we collect for this study will be stored for 5 years.

This study has been reviewed and given ethics approval by the Newfoundland and Labrador Health Research Ethics Authority. A few copies of the printed copy of the main consent form (3 pages) are kept beside the computer in a folder. We advise you to read before you participate in the study and you can keep one copy if you wish.

Therefore, anonymity and confidentiality cannot be fully guaranteed if you choose to participate in this survey.
