You’re invited to join a fun, free program to cook healthy meals with your family!
If you're picked for the Healthy Options at Home program, your family will get a meal kit once a week for 4 weeks. The meal kit has everything you need to cook healthy meals with one of our chefs.
We want to learn more about how you liked the program. To do this, we’ve teamed up with the University at Buffalo and Rutgers University. We’ll ask you to fill out 3-4 short surveys. One will be at the end of the program, and another will be 2-4 months later. Each survey will take about 15 minutes or less to complete.
You’ll get a $5 Tops gift card for each of the last 2 surveys, so $10 in total. After the final survey, you’ll also be entered into a drawing to win a kitchen tool of your choice worth $25 (like a blender or slow cooker).
We will keep your personal information private and safe. We will only share the results by combining everyone’s answers. No one will see your personal information.
The only risk is that someone might accidentally see your answers. But there is no cost to be part of the program. You don’t have to join if you don’t want to.
