Thank you so much for your interest in participating in the Centers for Disease Control and Prevention (CDC) Website Usability Testing project. The goal of this survey is to recruit participants who represent the variety of reasons visitors may have for accessing CDC's website. Please note that this survey is not the usability test, but serves as a way to help us recruit possible usability testing participants. Completion of this survey does not result in automatic eligibility for the usability testing.  If you are considered eligible for the testing you will be contacted individually using the contact information requested at the end of the survey.

If you are chosen to participate in the usability testing a session will be set up that may take about an hour where you will be asked to perform several short tasks using a website. You will also be asked questions about your experience and perceptions of the website. As an incentive for participation in the usability testing you will receive a $50 Visa gift card.
