Thank you for your interest in this survey.
The survey is being run by Coventry City Council to understand what people think about services which help people to make changes in their lives, such as stopping smoking or becoming more active. The results will be used to plan how lifestyle services could be updated in the future.
The survey should take approximately 10 minutes to complete and will be available to complete until 30th May. You can choose to be entered into a prize draw to win a £30 shopping voucher. If you wish to take part in the prize draw, please click on the link given at the end of the survey.
Coventry City Council will use your information to make sure that services to help people make changes to their lifestyles are useful for everyone, and to contact you about the prize draw or taking part in a further discussion. You do not have to take part in this survey. You only need to tell us personal information such as your name, address and telephone number if you want to enter the prize draw or take part in further research.
