Application Form

Help us improve health information! We usually ask people face-to-face for feedback on health information, but in light of COVID-19 social distancing measures, we need to do this in a new way.   

How it works
We’ll send you an email from time to time with new health information. All you have to do is read the information (it’s usually very short), fill out a form about the information and send it back to us.  

If you’d like to get involved, please fill in your details via this survey. We’ll be in touch! 

What will you do?
You can decide how much or how little you would like to do.
We will send you patient information resources and ask for your feedback on them. This includes flyers, information sheets, brochures and forms. We can send these in an email. We will send you a short feedback form to fill in about the resource.

What do we do with your personal information?
We may send you an email to ask for your feedback about health information up to once a week. We will only email you about health information. Only the Health Literacy team will have access to your personal information. We will not share your personal information or email address with anyone else, for any reason.
